The thorough plan for my project, which focuses on applying the health promotion model to address lifestyle risk factors contributing to hypertension in adults aged 25-45, was discussed during the threeway teleconference with my preceptor and faculty chair. DNP 855 Module 1 Assignemnt 1 described  how I identify patients in the acute care  unit at Roseland Community Hospital with the assistance of a carefully selected team of nurses.

These nurses will notify me when they locate patients who meet the target population so that we can offer targeted education on the risk factors for hypertension.
In order to improve patient comprehension of hypertension and nurse competency in providing health promotion, I intend to develop an educational bundle. The possible partnership with the hospital’s pharmacy to create a “meds to beds” program was also discussed.

DNP 855 Module 1 Prescribed Medications

In order to drastically lower hospital readmissions, this program attempts to guarantee that patients not only receive their prescribed medications prior to discharge, but also receive thorough instruction on how to use them. My suggested six-month project timeline was accepted by the faculty chair, who emphasized the significance of monitoring quantifiable results, especially within an eight-week period, in order to evaluate the intervention’s efficacy.
In order to clearly define the patient population, the issue, and the evidence-based intervention strategy, I was encouraged to start writing Chapter 1 of my manuscript. The teleconference ensured that my project was in line with academic and clinical goals by offering insightful advice and laying the groundwork for its subsequent phases.

Summary

The project’s main objective is to address lifestyle risk factors that contribute to hypertension in adults between the ages of 25 and 45 at Roseland Community Hospital by applying the Health Promotion Model. In the acute care unit, a group of nurses will determine which patients qualify, and the student will give them focused instruction on hypertension. In order to lower hospital readmissions, the project entails creating an educational package for nurses and patients as well as looking into collaborating with the pharmacy to launch a “meds to beds” program.
