Create Educational Modules for Newly Diagnosed Diabetes Patients
Measurable Goal
N550 Week 1 Assignment 3: Create educational modules for diabetes self-management, including nutrition, exercise, medication, and other activities.
Artifact/Evidence
All modules included Learning Outcomes, Evaluation, and Assets. These will be proven during a pilot meeting or concentrated when the authoritative subject matter expert feedback documents validate the educational content’s quality and credibility.
Establish and Maintain a Supportive Online Community for Diabetes Patients
Measurable Goal
Build and maintain an online communication and support channel for new diabetes patients through a social media package and/or discussion forum.
Artifact/Evidence
The snippets or links from the community depict proof of contribution and engagement. Members of the community provide proof by offering feedback that includes testimonials and examples describing how the community has overcome various challenges, thus showcasing the community’s impact.
Conduct a Diabetes Awareness Campaign in the Local Community
Measurable Goal
Arrange and execute a diabetes recognition campaign concentrated on the area people in the following two months.
Artifact/Evidence
Compile the campaign log, with the record of the activities, outreach, and the promotional materials used. Then, afterward, review and authenticate the implementation of the campaign alongside the network’s effect using the post attendance reports, media coverage, and campaign promotion reports.
