The disconnect between fragmented care and quality patient outcomes in the current era of healthcare data is typically the manner in which the information is gathered, analyzed and carried out in the point of care. As digital systems have increasingly been used by health care organizations, nursing informatics has emerged as a major specialty bridging the divide between clinical practice, information technology, and quality improvement.

The nurse informatics plays a significant role in transforming crude health data into meaningful outcomes that can be utilized in clinical decision-making, workflow, and patient outcomes. This paper is a proposal of a nursing informatics project that will improve patient outcomes and efficiency of care by implementing an advanced clinical decision support system (CDSS) within the electronic health record (EHR).

Description of the Proposed Nursing Informatics Project

The nursing informatics project proposed will cover the question of the most recent use of technology in the nursing care delivery, namely, the use of smartphones and telehealth to ensure that Medicare services are provided to more individuals through the use of telehealth-based service delivery. The proposed project is the introduction of the evidence-based clinical decision support system, which is a component of the current EHR system of the organization.

The CDSS will provide real-time notifications, alerts, and risk stratification tools to help nurses and providers with the early detection of at-risk patients, particularly those prone to medication errors, hospital-acquired infections, and clinical deterioration (Withall et al., 2022). The system will use the standardized clinical guidelines, data based on the patient, and predictive analytics to push timely interventions.

The project objectives are to reduce the variation in care delivery, ease the adherence to evidence-based practices, and simplify clinical work. The system minimizes interference in clinicians since it directly incorporates decision support into the normal clinician documentation procedures, thus enhancing accuracy and efficiency. To ensure the process of the CDSS customization reflects the nursing workflows, organizational goals, and promotes usability and adoption, respectively, nurse informaticists will lead the process (Bayor et al., 2025).

Stakeholders Impacted by the Project

The introduction of this nursing informatics project will have a few critical stakeholders who will be impacted. Bedside nurses, physicians, the advanced practice providers, as well as pharmacists, will be the key stakeholders as they will be dependent on the CDSS when making clinical decisions. System design, implementation, and maintenance will be targeted at nurse informaticists and information technology professionals. Quality improvement teams and patient safety officers will find better data visibility and performance measures beneficial.

It will be the leadership of the hospital as secondary stakeholders; they will have access to the information regarding the quality indicators of care and its efficiency, and patients will directly obtain safer, more coordinated, and timely care. Early interdisciplinary coordination and feedback will play a crucial role in ensuring that they share a common ownership and can put them into practice (Kaygan, 2022).

Technologies Required for Project Implementation

This project requires several integrated technologies to be implemented to be successful. The main technology of the organization is its EHR system, which is enhanced with a built-in clinical decision support system. The predictive analytics and data visualization tools will be used to analyze patient data and deliver it in the form of actionable insights. The seamless transfer of data between the systems and the departments will be improved with the help of HL7 and FHIR interoperability standards (Vishwasrao Salunkhe, 2024).

To enhance risk prediction and personalization of warning messages, it is possible to include artificial intelligence (AI) and machine learning algorithms. During the processing of large quantities of clinical data, these technologies are needed to see the trends, which may not be immediately obvious to clinicians. Secure data storage and cybersecurity are also necessary to safeguard patient data and ensure that regulations are followed (Abbasi and Smith, 2024)

Project Team and Role of the Nurse Informaticist

The project team will be a multidisciplinary team that will consist of a nurse informatics, a project manager, IT experts, clinical nurse leaders, physicians, pharmacists, and quality improvement experts. The nurse informatics will also be a key liaison point between clinical and technical staff since he or she will see to it that the system design is informed by the clinical requirements in the real world.

The nurse informatics will lead the workflow analysis, take part in the system configuration, training material writing, and will support change management. To ensure that the CDSS does not hinder clinical practice, the nurse informatics will ensure the nursing perspectives are incorporated in the system design. Continuous assessment and streamlining will also be assessed using nursing informatics competencies, which will help to sustain long-term impact (Eldoushy & Behairy, 2023).

Conclusion

Nursing informatics plays a crucial role in improving patient outcomes and the efficacy of care within the current healthcare organizations. The proposed clinical decision support project will be founded on technology utilization, interdisciplinary collaboration, and nursing knowledge to support improved clinical decision-making and patient safety. Since the evidence-based guidance is directly integrated into clinical workflows, the project enables timely interventions, minimization of errors, and high-quality care.
