To this point, I have completed 50 hours of my Outpatient Internal Medicine and Behavioral Health Integrated Care practicum at the Cleveland Clinic. My practicum immersed me in a field that has provided me with the ability to work with a large and varied adult clientele with acute and chronic medical and psychiatric problems. All of my practicum hours were done under the supervision of an authorized preceptor, and these hours were completed outside my employment.
I have completed a full assessment on a client. This includes client history, client physical exam, behavioral health assessment, and a review of the client’s medication history. I have taken part in the management and treatment of hypertension, type 2 diabetes, hyperlipidemia, anxiety, and major depression. I have learned to develop and construct differential diagnoses and health care treatment plans and strategies. I have also had the opportunity to learn numerous treatment plans in the behavioral health field.
My practicum has focused on the integration of behavioral health within primary care. I have completed depression and anxiety screenings, as the literature suggests. I have assessed clients’ medication management and developed coping strategies that incorporate the management of stress and related lifestyle changes. I have learned to accommodate both the behavioral health needs and the physical health needs of the clients.
A key advantage of my current role is that I collaborate with multiple professionals from the healthcare community, including physicians, advanced practice nurses, social workers, and pharmacists. Engagement in the case discussion collaboration team enhanced my understanding of the coordinated care planning process and my competency in the interpretation of diagnostic tests and procedures, along with establishing the follow-up care required by the patient.
The role of a clinician in me during the practicum was a great contributor to the enhancement of my clinical skills. I was able to work with an innovative health care system at the site that blended behavioral health with chronic health issues. This environment of diverse clinical care and strong collaboration extended the boundaries of advanced practice nursing.
